Food item are cooked to facility policy of 165F. Any leftovers are cooled in walk in refrigeration, in several pans, uncovered, no more than 3 inches deep. Discussed cooling schedule of 120F-70F in 2 hours, and then 70F-45F within an additional 4 hours. Staff was wearing glove while handling ready to eat food items, such as bread. All hand wash sinks were supplied with warm water, soap, and paper towels. Hands are to be washed for at least 20 seconds. Advised manager that hands are to be washed in between glove changes, when coming back to kitchen from break, and whenever hands are contaminated. Recommended that staff to be excluded from kitchen 24 hours since last food borne illness symptom. Symptoms include: nausea, vomiting, diarrhea, yellowing of the skin and eyes, fever, and open sores.

Observed 2 chlorine sanitizing solution buckets with wiping clothes, both at a concentration of ~100ppm, verified with facility's chlorine sanitizing test strips. Advised staff to use the test strips to verify that the concentration of the chlorine sanitizing solution is maintained at 100ppm or greater in buckets or at 50ppm or greater in the sink. Observed facility cooling chili con carne in one of the walk-in coolers. The chili con carne had a temperature of 37F. Advised staff to use an accurate probe thermometer to verify that time and temperature control for safety/potentially hazardous food (TCS/PHF) items are cooling 120F to 70F in 2 hours, and 70F to 45F in an additional 4 hours. Per interview with kitchen manager, foods to be cooled are stored in pans less than 4 inches in depth and placed in the walk-in cooler overnight uncovered before a lid is placed on the pan the next morning. Advised staff on proper hand-washing procedure using soap, paper towels, and warm water for 20 seconds. Advised staff to have an ill-worker policy and recommended excluding ill workers for 24 hours after the last sign of symptoms (nausea, vomiting, diarrhea, fever, yellowing of skin and eyes, and open sores).

Dataset Details

The New York State Department of Health's Bureau of Community Environmental Health and Food Protection works to protect the public health by assuring that food service establishments are operated in a manner that eliminates hazards through design and management, resulting in a decreased incidence of foodborne illness in our communities. The Bureau's Food Protection Program also coordinates foodborne outbreak investigations, analyzes the findings and uses this information to develop regulations and guidance designed to prevent similar outbreaks in the future. This data includes the name and location of food service establishments and the violations that were found at the time of their inspections. Violation details are collected on inspection reports (i.e., the actual food item, quantity and temperature of food found out of temperature control) as well as corrective actions for critical violations; this data set is limited to the violation number and the corresponding general violation description.

There are over 90,000 food service establishments in New York State. The Food Protection Program provides guidance and assistance to county and city health departments and State District Offices which in turn permit and inspect food service establishments, some institutional food services, temporary and mobile food service establishments and food and beverage vending machines. Each food service establishment is registered with facility name, business address, inspection dates, violations, etc.

To give New Yorkers the access they deserve to government data and information, Governor Andrew M. Cuomo launched the Open NY initiative in March 2013 and signed Executive Order 95. It directs state agencies to identify, catalog, and publish their data on the state's open data website administered by the Office of Information Technology Services (ITS). Open NY increases transparency, improves government performance, empowers New Yorkers to participate in government, and encourages research and economic opportunities statewide.
